• An affidavit, in the format, as per (Annexure-III) signed by the candidate and Parent in the presence of a Public Notary on a stamp paper of Rs. 200/- (To the Registrar)
• An affidavit, in the format, as per (Annexure-IV) signed by the candidate and Parent in the presence of a Public Notary on a stamp paper of Rs. 200/- (To the Principal)
• For GENERAL and NRI
• Submission of the following Original Documents is mandatory as per DGHS/MCC, at the time of admission
• Permanent MBBS Registration Certificate from any State Medical Council / Medical Council of India / National Medical Commission.
• Candidates claiming eligibility under SC / ST / Locomotor disability category and OBC as declared by the Government of India shall furnish the certificates to that effect issued by the competent authority as per DGHS / MCC guidelines. (Certified English version) (OBC candidates have to produce a Central OBC certificate).

Mandatory Documents submission by NRI/Foreign Candidates (as per MCC Notice):

NRI / NRI ward/ OCI / PIO / Foreign Nationals Candidates:

1. Affidavit of the person who is NRI and the sponsor.

2. Documents showing that the sponsor is an NRI ( Passport, Visa of the sponsor)

3. Candidate affidavit showing the relationship of NRI with the candidate as per the court orders of The Honorable Supreme Court of India in the case (W.P.(C) No. 689/2017 – Consortium of Deemed Universities in Karnataka (CODEUNIK) & Anr. Vs. Union of India & Ors) dated 22-08-2017.

4. Affidavit from the sponsor that he/she will sponsor the entire course fee of the candidate.

5. Embassy Certificate of the Sponsor (Certificate from the Consulate issued within the last 06 months).

6. Family tree notarized by tahsildar

7. Income Tax Documents are required as per the Income Tax Act 1961.

8. The requisite fees have to be paid from the Sponsor's (Foreign / Indian NRE) Bank account only

For Foreign Nationals:

• Foreign national’s qualification registration certificate by the corresponding Medical Council or concerned authority in his / her own country from where he / she has obtained his / her basic medical graduate qualification.
• Obtain a Temporary registration certificate from MCI/DCI within one month of securing admission.
