For UG Candidates

• Admit Card of NEET exam.
• Report card or Rank Letter.
• Class 10 certificate and marksheet for the birth proof.
• Class 12 certificate and mark sheet
• ID proof as in Aadhar/PAN Card/Driving License/Passport.
• Eight copies of passport-size photographs
• Provisional Allotment Letter
• Caste Certificate (if applicable)
• PwD Certificate (if applicable)

For PG Candidates

Original Copies Of:

• MBBS Certificate.
• 3rd Part II mark sheet of MBBS degree.
• Certificate of Medical Registration.
• Certificate for Internship Completion. A certificate with an attested photo from the head of the institution last attended or a copy of a passport, an Aadhar card, a PAN card, or a driver's licence (self-attested photocopy after checking the original papers) are acceptable forms of photo identification.
• If the date of birth is not listed on the HSC mark sheet or certificate, a copy of your birth certificate is required as evidence of age.
• A copy of the state government's sponsorship certificate and your registration for online counseling, if appropriate.
• 8 copies of passport-size photographs.
